The Federal Aviation Administration (FAA) issued its own and similar directive, AD 2018-18-01, on October 1. The initial CFM service bulletin and airworthiness directives were issued as a result of the highly publicized fan blade failure on Southwest Airlines Flight 1380, a 737NG departing from New York - LaGuardia Airport on April 17, 2018. The event resulted in one passenger fatality, serious damage to the aircraft from flying debris, and a dramatic emergency landing at Philadelphia International Airport.
